New Client Forms
Prior to your first appointment, please download these forms, fill them out and have them ready for your first appointment.
Counseling fees are $120/clinical hour (a clinical hour = 50 minutes). Actual client fees will be determined at the first counseling session, which is free for each new client. Sometimes counseling sessions extend beyond the clinical hour at the discretion of the counselor, at which point you will not be charged for the additional time.
Counseling Fee Schedule
The client shall make full payment at the beginning of each session unless other arrangements have been made prior to the appointment.
The APS Profile is a separate charge, and must be paid for in the initial meeting, as you fill out the APS Form. The APS will provide valuable information about your personal Temperament, and will be a great asset in understanding your needs. The APS Profile fee for each person is $60.00. The profile fee for couples is $110.00 total — when taken together. Your normal Session Fee will begin with the second session.
Payment is accepted by cash, check or credit card. We do not extend credit or schedule appointments beyond one unpaid session until payment is made. Clients are fully responsible for the payment of all fees.
If there are financial concerns that make your fee too difficult to pay, we will discuss other alternatives. The goal is to find a set amount you can manage so that you can get the counseling help you need.
Counseling by Live Video
If you are not able to meet in person, you can set up an appointment to meet with Dr. Daniel R. Sirstad through your computer using Thera-LINK. The Thera-LINK platform keeps our conversation secure and HIPAA Certified.
With video capabilities, no matter where you live in the world, you can have a personal, private counseling session with Dr. Sirstad. Contact us to learn more.
You may want to check with your insurance company to see if mental health therapy is covered. Clients understand that a Pastoral Counselor will not file on insurance — clients are responsible to bill their own insurance if they believe a Pastoral Counselor is covered.
